Midtown Community Health Center takes pride in serving everyone; we make quality healthcare accessible. We provide a full spectrum of affordable healthcare to community members with and without medical insurance, and from different cultures and backgrounds. We provide translation services in many languages.

We accept the following forms of insurances:

  • Medicaid
  • Medicare
  • CHIP (Children’s Health Insurance Program)
  • PCN (Primary Care Network)
  • Most private medical insurances (please confirm when you set an appointment)

* Patients without medical insurance are welcome and will be charged at the time of service on a discounted sliding fee scale based on income and family size. A Good Faith Estimate is provided at the time a patient schedules an appointment through text. A patient may call a Patient Accounts Representative (801-393-5355) or ask a receptionist for a Good Faith Estimate.

Midtown Community Health Center will not turn away a patient for their inability to pay.

        • Certified letter of income from your employer(s)
        • Unemployment benefits statement
        • Social Security, disability or pension benefit statements
        • Documentation of other government assistance
