Since 1994, Midtown Community Health Center has provided essential healthcare to community residents in Northern Utah — especially to our underserved populations. Now with seven healthcare facilities, our family of clinics provide medical, dental, behavioral health and pharmaceutical services to over 26,000 patients a year. We are proud to make a difference in our community and believe that every person should have access to personalized, high-quality, and affordable healthcare.
Midtown operates as a private, non-profit community healthcare center and accepts patients with and without private health insurance*, as well as Medicaid, Medicare, CHIP (Children’s Health Insurance Program), and PCN (Primary Care Network).
We serve patients of all ages and cultural backgrounds, offer translation and coordinated-care services, and can help you explore your healthcare options to determine if you are eligible for government programs/services or other financial assistance programs. As your primary care provider, Midtown will coordinate all of your family’s needs under one roof, in a comfortable, modern, and friendly setting.
In addition to primary healthcare services, Midtown also provides the following services: prenatal care and delivery, well child visits, women’s health, geriatrics, treatment for acute and chronic illnesses, and infectious disease management. Midtown Community Health Center in Ogden offers a walk-in clinic for acute illnesses such as infections and minor emergency care for conditions like lacerations and sprains.
* Patients without medical insurance are welcome and will be charged at the time of service on a discounted sliding fee scale based on income and family size.

Midtown’s clinics are staffed with dedicated physicians, physician assistants, nurse practitioners, dentists, psychiatrists, mental health professionals and pharmacists. Our team not only provides excellent healthcare services, they are also dedicated to Midtown’s mission of embracing and providing care to everyone in the community — regardless of background or circumstances. Services are available in English and Spanish; translation services are also available in any language.
