Review a summary of your most recent visit. You can view each visit summary for your appointment.

In Person at One of our Locations:
Ask any member of the front desk staff to enable your account for the Patient Portal. You will be asked to verify your identity and provide a valid email address.
Over the Phone:
Call 801-393-5355 and ask our patient services representative to enable your Patient Portal. You will be asked to verify your identity and provide a valid email address. You will need to contact us again if you do not activate your account within 48 hours of receiving the email.
Please call us to enable your patient portal then download the Athena Patient app through your Apple or Google Play account to access your health information.
