You can review and download portions of your health record. If you need a complete copy of your personal health record, please call our clinic.

Review, Download, or Print Your Visit Summary

Review a summary of your most recent visit. Access your visit summary by tapping on the My Records icon. You can view each visit summary for your appointment.

Request a Medication Refill

Review a list of your current medications and request a refill. In your Meds Cabinet, click “+” icon in the top-right corner and select Request Refill.

+-How do I sign up for the Midtown Patient Portal?

In Person at One of our Locations:

Ask any member of the front desk staff to enable your account for the Midtown Clinic Patient Portal. You will be asked to verify your identity and provide a valid email address.

Over the Phone:

Call 801-393-5355 and ask our patient services representative to enable your Midtown Clinic Patient Portal. You will be asked to verify your identity and provide a valid email address. You will receive an email with a temporary password and activation link. You will need to contact us again if you do not activate your account within 48 hours of receiving the email.

After enabling your Midtown Patient Portal, download the Healow app through your Apple or Google Play account to access your health information.
