Tyler Farnsworth, DNP, PMHNP-BC

Midtown CHC of Davis, South Salt Lake
Psychiatry, Substance Use Disorder

Tyler completed his Doctor of Nursing Practice at Ohio State University, specializing as a Psychiatric Mental Health Nurse Practitioner. Tyler also completed a year-long Advanced Practice Provider Fellowship in Child & Adolescent Psychiatry at Primary Children’s Hospital, where he received advanced training in caring for young patients and their families. He is passionate about providing compassionate, evidence-based care to patients across the lifespan, with a particular interest in supporting individuals struggling with substance use disorders. Tyler believes in creating a collaborative, respectful environment where patients feel heard, understood, and empowered in their treatment journey.
Outside of work, Tyler enjoys skiing, hiking, paddle boarding, and camping with his two Australian Shepherds. His favorite travel assignment of all time was in Alaska — which is still his favorite place in the world. He returns every year. It was during his time caring for veterans and individuals struggling with substance use disorders, particularly in Alaska, that he decided to pursue his Psychiatric Mental Health Nurse Practitioner degree. His original plan was to work at the VA. However, during clinical rotations working with children and adolescents in inpatient, outpatient, and residential settings, he found the work so meaningful that he decided to pursue additional specialized training to better serve kids and families.
