Dr.Haydee Blanca, MD
Dr. Haydee L. Blanca is a Board certified pediatrician from Havana , Cuba who has studied and practiced in different countries like Cuba and Chile and now here in the United States, both in New York and Utah. She graduated from Medical School in Havana and retook her studies in Santiago de Chile, Chile and later on did a Pediatric Residency at Bronx- Lebanon Hospital Center NY and The Children Hospital at Montefiore NY under Albert Einstein College of Medicine of Yeshiva University (Bronx -Lebanon Program). Her inviting everyday manner, and genuine concern for her patients, combined with evidenced-based medicine allows Dr. Blanca to ensure that all of her patients secure a healthy and meaningful start in life. Dr. Blanca has been practicing at Midtown Community Health Center, Children Clinic since 2009. When she is not seeing patients, she enjoys hiking, reading and traveling with her husband and their Three Princesses- Isabella, Haydee, and Paula.
Colby Andersen, PA-C
It's great to be back in Cache valley, I grew up near Logan and it's exciting to get back to the mountains for hiking and camping.
My wife and 3 kids are excited to able to hang out with cousins more and help out on the family dairy farm.
I graduated from PA school in 2010 from Alderson-Broaddus University in West Virginia, spent my first 3 years in Colorado and the last 5 years in Boise area.
Vance Anderson LCSW
Vance Anderson graduated from Weber State University in 1986 with a major in Social Work. He worked for Weber Human Services in 1973 and earned a master’s degree in social work from the University of Utah in 1997. Vance has spent his career with Weber Human Services and McKay Dee Hospital’s psychiatric unit. Vance has been very active in his volunteering throughout the community serving on several boards, coaching youth wheelchair basketball and traveling to many countries representing the USA in wheelchair marathons. Vance completed the Boston Marathon four times placing as high as 3rd. In 1989 he received an award for Gov. Scott Matheson for Utah’s outstanding handicapped citizen of the year. Vance is our LCSW at our Hope site where he counsels and does group work with individuals experiencing homelessness.
Dr. Richard Arbogast, MD
Dr. Richard Arbogast graduated from Johns Hopkins Medical School in 1964. He did a fellowship in Internal Medicine from 1964-1965, and a residency in psychiatry at the University of Cincinnati from 1965-1968. Dr. Arbogast completed a second fellowship in community psychiatry at the National Institute of Mental Health from 1968-1970. He is board certified by the American Board of Medical Management, in Family Medicine and Psychiatry and Neurology. Dr. Arbogast helped establish some of the first medical services offered to individuals experiencing homelessness in Weber County and has volunteered for many years at Seager Memorial Clinic. Dr. Arbogast retired from his role as medical administrator for IHC 3 years ago and has been doing part-time clinical work in family medicine and psychiatry since. He feels lucky to have the opportunity to work at the Hope site.
Van Aston, PA-C
Van Aston attended Weber State University for his undergraduate degree in Clinical Laboratory Science, graduating in 2007. He attended Emory University School of Medicine graduating in 2011 with a Masters of Medical Science degree as a Physician Assistant. He has practiced rural and community medicine since graduation. He is certified to practice medicine by the National Commission on Certification of Physician Assistants. In addition to work, he is actively involved volunteering in the community and spends as much of his free time as possible outdoors and with his family.
Parker Beus, MSW, CSW
Parker Beus received his undergraduate in Social Work at Utah State University and his Master's in Social Work at Brigham Young University. He has experience working with teenagers at the Utah State Hospital in Provo, and works in private practice with sexual addiction and trauma.
Tyler Coe, FNP
Tyler Coe has been a practicing Nurse Practitioner since 2015 and has loved every minute of it. He received his Master’s degree in Nursing as well as his Family Nurse Practitioner license at Grand Canyon University in Phoenix, Arizona. Tyler loves Scouting and has been a scoutmaster for the last 9 years. During this time, Tyler has had a total of 22 Eagle Scouts he has had the pleasure of teaching, his son will make 23. Tyler loves doing anything that involves his bride and 4 kiddos, "you name it and we will do it, or have done it". Tyler loves helping people anyway he can and feels so blessed to be in a profession where he is able to do just that.
Kelson Crowther, PsyD
Kelson received his undergraduate degree from Weber State University following his service as a missionary for the Church of Jesus Christ of Latter Day Saints in Guadalajara Mexico. He continued his education at Pacific University in Hillsboro, Oregon where he received his doctorate in Clinical Psychology with a concentration in Industrial-Organizational Psychology and Latino Bilingual Psychology. He completed his APA-accredited internship at Providence Family Medicine Center in Anchorage, Alaska where he developed his passion for Behavioral Health Consultation in an integrated primary care setting. Kelson is an avid outdoors-man and enjoys spending quality time with his family and friends.
Dr. Daniel McConkie, DDS
Dr. Daniel McConkie graduated from Marquette University School of Dentistry in 2007. After graduation, he began working in West Valley where he realized how much he had left to learn! Working with world class doctors helped him not only make the transition from the controlled environment of a dental school to the real world challenges that dentistry can provide, but also realize that ” average” dentistry just isn’t good enough. It has given him the goal of producing dentistry that makes other dentists say “Wow. I want him to be my dentist.” He then moved to his own practice in the Ogden area and loves working in this beautiful city. Dr. McConkie and his wife are proud parents of four beautiful daughters and one son; they love the outdoors and spending time together. He also loves his job and appreciates the trust that his patients place in him. He is thankful for the chance to serve you.
Amanda Gougler, PA-C
Amanda is a board certified Physician Assistant, who started at Midtown in September 2015. She grew up in Layton, and studied Public Health at Brigham Young University with a focus on health education and preventative care. She graduated from Midwestern University in Glendale Arizona, and is so glad to be back in Utah. She loves hiking, playing the piano, and is working at improving her limited Spanish. She and her husband have been married for three years, and they have a sweet puppy the hope to train as a therapy dog.
Dr. Stanley Graham
Dr. Stanley Graham is a board certified family physician who started working at Midtown in 2011. Dr. Graham obtained his Bachelor of Science degree in Molecular Biology from Brigham Young University in 1998, as well as his Master’s degree in Molecular Biology in 2002. He completed his residency program at McKay-Dee Family Residency Program in 2009. Dr. Graham enjoys serving his patients at Midtown, as well as spending quality time with his wife and two beautiful children.
Dr. Richard Gregoire
Dr. Richard Gregoire has been practicing medicine since 1994. He graduated from the State University of New York at Buffalo School of Medicine and completed his residency at the University of Utah Department of Family and Preventive Medicine. He also has a PhD in Biomedical Sciences received in 1988 from the State University of New York at Buffalo. He has worked at Midtown Community Health Center since 1994 and served as Medical Co-Director from 1995 to 2013.
Nate Hadley, PhD, MBA, MSW, LCSW
Nate has been with Midtown for almost 15 years and oversees the mental health services department. He also provides mental health counseling for individuals, couples and families. He assists patients in addressing a wide variety of mental health issues and conditions. Nate enjoys his career as a counselor as it allows him to help others overcome difficult issues they are facing in their lives and to assist them in achieving their personal goals. Nate has a PhD in Social work from the University of Utah. He also has Master’s degrees in Business Administration and in Social Work. He also has a Bachelor’s degree in Spanish Language and Family Studies. Nate enjoys spending his free time with his family and loves playing sports, being in the outdoors, studying Zen philosophy and speaking the Spanish language.
Tiffanie Haun, FNP, Medical Co- Director
Tiffanie Haun is an AANP certified Family Nurse Practitioner who has worked at Midtown since 2005. She studied nursing at Weber State University and also at the University of Utah. In addition, she obtained a bachelor’s degree in Zoology prior to going on with her studies in nursing. She enjoys spending time with her family, skiing, boating and playing all kinds of sports. Her areas of interest are Family Practice and Women's Health. She is fluent in Spanish and loves serving her patients at Midtown and looks forward to many more years to come.
Dr. Mark Housley
Dr. Mark Housley is board certified in family medicine. He studied medicine at the University of Utah and completed his residency program at McKay Dee Hospital, graduating in 2005. He enjoys all aspects of family medicine. He loves to care for his patients through their pregnancies and then watch as their children grow. He also enjoys teaching resident physicians in training.
Vanessa Jones, FNP
Vanessa was born in Santiago Chile and grew up in California. She worked at Ronald Regan UCLA Medical Center for nine years as a Trauma Certified Registered Nurse in the areas of the Emergency Room and Intensive Care. She moved to Utah in 2013 to be close to family. In 2015 Vanessa decided to further her career as a Family Nurse Practitioner at Maryville University where she formally graduated. Vanessa enjoys music, acting and loves boating with her husband and kids. Her professional interests are family medicine and urgent care. She speaks fluent Spanish and Italian. Vanessa is very excited to be part of the Midtown team and looks forward to being a part of this outstanding team for many years.
Brad Larsen, PA-C
Brad completed his education as a physician assistant in the Spring of 2000 at Des Moines University in Iowa. He has devoted the majority of his career working with the under-served in community health and urgent care settings in Iowa, Texas, and Utah. He was with us here at Midtown from the Summer of 2008 to the Spring of 2015. During the last 3 years he has been working with the University of Utah at the Huntsman Cancer Center, and helping in the community urgent care clinics. Brad has returned to be with us where he would like to continue to contribute to the Midtown mission. He loves to hike, camp, fish, play golf, and snow ski. His favorite hobby is playing old 1970’s and 80’s songs on his guitar at home, or wherever he is tolerated.
Heather Larson, PA-C
Heather is a new Physician Assistant at Midtown as of October, 2017. She received her Master’s degree in Clinical Health Services from the University of Washington with an emphasis on underserved primary care. Her undergraduate degree was received from Weber State University. Heather will take every opportunity to travel and has spent time in 15 different countries so far. She also enjoys rock climbing with her husband, reading and spending time with her family. Heather is also working on improving her limited Spanish and is excited for the opportunity to serve her patients at Midtown.
Dr. Carissa Monroy, MD
Carissa Monroy is a family physician at Midtown in South Salt Lake. She is from Salt Lake City, attended college at Stanford University, and completed her medical training at the University Of Utah School Of Medicine, where she received Masters in Public Health as well. She completed residency at McKay Dee Family Medicine Residency in Ogden. Dr. Monroy sees children and adults, and her interests include hepatitis C treatment, adolescent medicine, refugee health and women’s health. Dr. Monroy is married to another family physician, and they have 3 children. She spends her free time trying to keep the house in order, skiing/snowboarding, hiking, shopping and attempting to do crosswords.
Justine Murray, MSW, CSW
Justine is a graduate from Boise State University with a Master’s of Social Work. She also has two Bachelor’s degrees from Weber State University in Social Work and Criminal Justice. Justine has been working with people experiencing homelessness since 2013. Currently Justine works as the Executive Manager for Youth Futures a homeless youth shelter, and works at the Midtown HOPE Clinic providing therapeutic group services and crisis intervention.
Derek Petersen, PA-C
Derek Petersen is a board certified Physician Assistant who joined the Midtown team in the Spring of 2017. He received his master’s degree in Physician Assistant Studies from the University of Dayton Ohio. He completed his undergraduate studies in Health Promotion and Education at the University of Utah. In his spare time he enjoys hiking, biking snowboarding and wake boarding. He couldn’t wait to return to his hometown of Ogden, Utah to start his family. He is passionate about caring for the pediatric patient and grateful for the opportunity to be a part of the Midtown community.
Cristina Regalado, PA-C
Cristina Regalado is an NCCPA board certified Physician Assistant at Midtown in South Salt Lake. She began working at Midtown in 2015. She is from Dallas, Texas and attended college at University of Texas at Dallas, where she received her Bachelor’s degree in Biology. She completed her medical training at the University of St. Francis Physician Assistant Program in Albuquerque, NM. Prior to becoming a Physician Assistant, she was a Physical Therapy Technician. Cristina sees children and adults, and has a strong interest in Women’s health. Cristina spends her free time running, hiking, rock climbing, and doing yoga.
Pilar Reyna, CSW
Pilar earned her Master of Social Work degree from Florida International University in Miami, Florida in 2008, and her Bachelor of Arts degree in Psychology from California State University, San Marcos, in 1998. Pilar arrived at her mental health practice later in her life after spending several years working in child protective services and adult probation. Her professional journey has brought her to Midtown where she hopes to be a source of support to her clients. Pilar enjoys watching movies (a lot), reading, spending time with friends and family, and spending tons of quality time with her two cats, Neo and Johanna. Spanish is Pilar’s native language.
Dr. Kurt Rifleman, MD
Dr. Kurt Rifleman is a board certified family physician, who has spent his entire medical career at Midtown, beginning in 1998. He studied medicine at the University of Utah and trained for family practice at the McKay Dee FP Residency in Ogden, UT. He enjoys serving in volunteer positions both in the community and in organized medicine, teaching medical residents and students, and exploring anything and everything. But primarily, he wants to serve patients at Midtown. It is often assumed that he is fishing when he doesn’t answer the phone. He loves to treasure hunt with his wife and two children. He is also a volunteer for the Boy scouts of America.
Sarimin Rivera-Llavona, RD
Sarimin graduated with a B.S. Nutrition and Dietetics from the University of Puerto Rico in 2005. She completed her Dietetic Internship at SeaMar Community Health Center in Seattle, WA. Sarimin has been working as a Registered Dietitian for about 12 years. Her area of practice while in Washington State was mostly in Pediatric in/out patient. She has advanced training in nutrition for children with special health care needs. Worked 8 years for WIC Nutrition Program, and became a Certified Lactation Educator. Now in Utah, she is happy to work for Midtown Community Health Center, where she is committed with Diabetes Education and Chronic Disease Management and Prevention. She has recently completed advanced training in Weight Management offered by the Commission on Dietetic Registration. Currently working on her CDE certification. “I truly enjoy and hope to help improving the access to diabetes care and health education, especially to those undeserved. She is bilingual English/Spanish. Loves music, traveling, dancing, cooking, gardening, kayaking and exploring the nature.
Alex Suarez, MSW, LCSW
Alex received his undergraduate degree in Social work and Sociology from Weber State University and continued his education at Texas A&M-Commerce University where he graduated with a Masters in Social Work in 2007. Alex has had the opportunity of working in a variety of areas in his field which have also included Domestic Violence, Sexual offenses and addiction issues. Alex grew up in the South Florida area and has lived in Idaho and Utah for the better half of twenty years. Alex speaks Spanish and enjoys the interactions he has with his clients while providing bilingual therapy services to a diverse population. He enjoys spending his free time with his three children and traveling when possible.
James Sutton PA-C
James “Jim” Sutton was born and raised in Seattle, WA but moved to Utah for school on to where he met his wife. He graduated with his Bachelor’s degree at Utah Valley University and then completed the Physician Assistant Program at Idaho State University in Pocatello, ID. While Jim was in PA school, he had the opportunity to assist with the delivery of his twins. Jim and his wife both come from very large families and they have ten children together.
Jim loves to spend time with family watching football, river rafting, camping and biking.
Darrin Tuckett, APRN
Darrin Tuckett is a Family Nurse Practitioner who started working at Midtown in 2013. Darrin obtained his Bachelor of Science degree in Nursing from Weber State University in 2011, as well as his Master’s degree in Nursing, Family Nurse Practitioner from Westminster College in 2013. Darrin provides services at our South Salt Lake clinic. Darrin is an avid runner and enjoys competing in several running races and marathons, as well as spending time with his family.
Lorena Villarreal de Cantu, PA-C
Lorena joined Midtown as a pediatrician at our Children’s Clinic located in Washington Terrace, UT in November 2011. She is a NCCPA Certified Physician Assistant. She graduated from the University of Utah, Physician Assistant Program in 2011. She obtained her Bachelors in Science degree in Microbiology from Weber State University, from where she also studied English as a second language. Lorena studied medicine in Mexico from 1983 to 1990 and completed a residency program in radiology from 1991 to 1994. She came to live in the USA when she married in 1996. She has always felt attracted to pediatrics and her desire to practice medicine with children finally came true when she joined Midtown. “My call is to be with the little ones, especially with the underserved, younger patients”.
Ryan Wojciechowski, PA-C
Ryan Wojciechowski has been a licensed Physician Assistant since 2006. He graduated from the University of Washington MEDEX Physician Assistant program and completed his undergraduate degree at Weber State University. He has clinical experience in sports medicine, emergency medicine, primary care and urgent care. Ryan enjoys snowboarding, hiking, exercising, listening to music and spending time with his family.
