What Do I Bring to My Appointment?

Midtown accepts Medicaid, Medicare, CHIP, and all major health insurance plans. A sliding fee scale is available for individuals who do not have health insurance. Discounted fees are determined by your family size and income. Therefore, please bring the following with you:

• Insurance card
• Current paycheck stub or tax documents
• List of medications you currently take
• Immunization record
• Minimum payment of $25
