Alicia Martinez

Chief Executive Officer

Alicia has worked with Midtown Community Health Center since 2001. She has held multiple positions within the organization including medical assistant team lead, quality improvement assistant, center manager, and chief operations officer. She has assisted in the planning and opening of 6 Midtown Community Health Center locations, and has managed multiple locations over the last 10 years. As the executive director, Alicia works with staff and community partners to improve the quality of the healthcare provided to our community residents.


Jesse Bush

MPH, CPH, MCHES

Chief Operations Officer

Jesse received his B.S. and Master’s degree in Public Health (MPH) from Brigham Young University. He comes to us from Public Health, formerly working at local health departments for the past 9 years. He focused on implementing primary prevention policies/programs (chronic disease prevention, injury prevention, tobacco cessation and prevention, as well as healthy lifestyles for teens) with state and local stakeholders. He is interested in promoting inclusion in community settings and increasing access to equitable and affordable healthcare.


Tiffanie Haun

FNP

Chief Medical Co-Director

Tiffanie Haun is an AANP certified Family Nurse Practitioner who has worked at Midtown since 2005. She studied nursing at Weber State University and also at the University of Utah. In addition, she obtained a bachelor’s degree in Zoology prior to going on with her studies in nursing. She enjoys spending time with her family, skiing, boating and playing all kinds of sports. Her areas of interest are Family Practice and Women’s Health. She is fluent in Spanish and loves serving her patients at Midtown and looks forward to many more years to come.


Dr. Kurt Rifleman

MD

Chief Medical Co-Director

Kurt Rifleman, MD is a board-certified family physician who first came to Midtown Community Health Center in 1998 as a new physician and has spent his entire career at the center. He studied medicine at the University of Utah and trained for family practice at McKay Dee FP Residency in Ogden. He enjoys serving in volunteer positions both in the community and in organized medicine, teaching medical residents and students, and exploring anything and everything.


Jeanette Colvin
Chief Information Officer

Jeanette joined Midtown Community Health Center in a part-time capacity in 2000 and became the Chief Information Officer in 2010. She has worked for non-profit organizations for the past 10-years, managing all facets of information technology.


Sonja Levesque
Chief Financial Officer

Sonja has been the CFO of Midtown Community Health Center since June 2003. She had previously worked for the Center and Planned Parenthood Association of Utah in various accounting positions for 10 years. Sonja graduated from Southern Utah University with a Bachelor of Science in Accounting and Business Management.


Braden Mitchell

PharmD

Pharmacy Director

Braden is a graduate of the University of Utah with a Bachelor’s of Science degree in Pharmacy.  He also has a Bachelor’s degree in Microbiology from Weber State University. He has been working in community pharmacy in the Ogden area for the majority of his career. He has a strong belief in community and public service and is currently serving on the Riverdale City Council.  He has been with Midtown Pharmacy since it opened and is currently the Pharmacy Director.


Brock Rose

DMD

Dental Director

Brock Rose, DMD received his bachelor’s degree in Exercise Science from Brigham Young University and graduated from Roseman University of Health Sciences College of Dental Medicine in 2019. He began working at Midtown Community Health Center immediately after graduating from Dental School. He enjoys all aspects of Dentistry and believes in providing the highest quality of care to his patients.


Kelson Crowther

PsyD

Behavioral Health Director

Kelson studied psychology at Pacific University in Hillsboro Oregon, where he received his doctorate in Clinical Psychology with a concentration in Industrial-Organizational Psychology and Latino Bilingual Psychology. After completing his internship at Providence Family Medicine Center in Anchorage Alaska, Kelson moved back home and has worked at Midtown ever since. Kelson is both a therapist and Behavioral Health Consultant, providing services in English and Spanish. He enjoys working with Midtown’s amazing patients, is an avid outdoorsman, and loves spending quality time with family and friends.


Jayme Hyatt
Business Office Manager

Jayme began working for Midtown Community Health Center as the Business Office Manager in April 2009. She has 13 years of management experience and 7 years of experience in the healthcare industry working with insurance companies and patients.


Janessa Fernandez
Human Resources Director

Janessa started as a patient service representative at Midtown in 2015, then became an outreach and enrollment specialist and HR assistant. Prior to Midtown, she spent several years working in public health for the federal WIC program in Logan. She plays a pivotal role in the development and implementation of processes aimed to improve training, quality and employee satisfaction. With continued education, she hopes to become SHRM certified. As a single mother, Janessa is dedicated to raising her two daughters, and values the time spent with her family and friends.
